Please complete this form if you undertook Nursing, Midwifery or Physiotherapy (including Sport Rehab) training at the School of Health Sciences at The University of Nottingham (or former Institution) graduating between 2002-2019 and require a transcript of your training.

NOTE:  The University is only able to provide a standard transcript of marks.  A transcript constitutes a list of modules and results (pass or fail) and marks where available.  Details of clinical attachments will NOT be included.  The transcript does not show credit weightings or dates of completion.  The degree title is listed at the top of the transcript along with the graduate's name and NMC PIN number.  

An electronic Graduate Transcript may only be issued directly to the third party organisation requiring this document.

The recipient details will then be verified via the organisation’s website, if the verification cannot be determined then this will be referred back to the originator of the order.

Prospective employers should use HEDD (Higher Education Degree Datacheck) to verify an award gained from the University of Nottingham.  

Transcripts can take up to 28 days to process.  

Important information regarding programmes delivered by Mid-Trent College

The University of Nottingham, School of Health Sciences no longer hold detailed records of programmes of study delivered in the 1990s by Mid-Trent College.  As we are the keeper, not the originator of this data, it is therefore not possible to produce the level of information requested relating to certification, module content or module marks.  

The University cannot provide any other 'non transcript' records relating to Mid-Trent College graduates, nor confirm their programme of study or award.  

Mid-Trent College graduates who request confirmation of their Mid-Trent College award or qualification should be directed to the Nursing and Midwifery Council who will be able to confirm that they joined the nursing register on completion of their qualification and therefore completed their training with an appropriate institution.  Please refer to this link.
