Nursing, Midwifery and Physiotherapy including Sport Rehab Transcript of TrainingDescriptionThose graduating between 2002 and 2019 Please complete this form to request a transcript if you studied Nursing, Midwifery or Physiotherapy at University of Nottingham (or a former institution) and graduated between 2002 and 2019. The University will provide a transcript of the modules completed, the results (pass or fail) and the marks where available. The transcript will show the graduate’s name and degree title, along with their NMC PIN number (where relevant). The transcript will not include details of clinical attachments or placements, and will not show credit weightings or dates of completion. Transcripts may take up to 28 days to process. Electronic transcripts Please note that an electronic transcript can only be issued directly to the third party organisation requiring the document. If you require an electronic transcript, please select the email option on the form and ensure you provide the full details of the organisation that the transcript should be sent to, along with their correct email address. The recipient details will be verified before the transcript is sent by email. If the email address cannot be verified then your transcript request will not be fulfilled. If a prospective employer wishes to verify an award gained from University of Nottingham they can do so using the HEDD (Higher Education Degree Datacheck) service. Those graduating in 2001 and earlier The University is not able to provide a transcript of marks for those graduating in 2001 or earlier. For those graduating in 2001 or earlier, you can request a letter to confirm your degree, classification (if applicable) and study dates by clicking here. Former students of Mid-Trent College University of Nottingham no longer hold detailed records of programmes of study delivered in the 1990s by Mid-Trent College. The University cannot provide any transcripts or other records relating to Mid-Trent College graduates, nor confirm their programme of study or award. Graduates of Mid-Trent College who need confirmation of their qualification should contact the Nursing and Midwifery Council who will be able to confirm the date they joined the nursing register.
